Post Mortems (in IT)Man am I a huge fan of the post mortem. How the hell do you attempt to avoid making the same mistake twice if you don't identify it the first time? I know why people are reluctant to do them... it's very tough to avoid blaming people, but trust me, you and your org are better for it.D
